Potential Problem:
Breg is recalling Kodiak Power Supplies used with Kodiak cold therapy units.  Use of these supplies has the potential to result in shock, fire or loss of power.

Dates Produced:  11/2008 – 6/10/2009

Models Affected:  AD-0660G with lot codes 3508, 3908, 4308 or 4808

Solution:
Breg will notify customers. They are instructed to quarantine the Power Supply and then destroy and dispose of them.  Replacement Power Supplies will be provided by Breg.  Customers may contact Breg Customer Relations @1-800-321-0607
